- פן קדמי: דיוקן אלי כהן, ומסביב "אלי כהן · מסר נפשו למען העם והמדינה · ט"ז אייר תשכ"ה", ולמטה "NATIONAL HERO ELY COHEN". לצד הדיוקן מגילה עם הכיתוב "יצאת לישע עמך".
- פן אחורי: הכותל המערבי ודגל ישראל, עם הפסוק "לגבר אשר דרכו נסתרה · ישב בדד וידום כי בטל עליו" (איוב ג, כג) / "TO A MAN WHOSE WAY IS HID".

A private commemorative medal in memory of the Israeli hero Eli Cohen. Made of gold-plated copper, 6 cm in diameter.
- Obverse: portrait of Eli Cohen, encircled by "Eli Cohen · gave his life for the people and the state · 16 Iyar 5725", and below "NATIONAL HERO ELY COHEN"; beside the portrait a scroll reading "Thou wentest forth for the salvation of thy people".
- Reverse: the Western Wall and the flag of Israel, with the verse "To a man whose way is hid · he sitteth alone and keepeth silence" (Job 3:23).
